What doctors should compare before choosing a physician loan
A specialty mortgage can read a medical career more accurately. It still needs to win the full cost and flexibility comparison.

The physician-loan label is not the comparison
Physician mortgage programs are portfolio products created by individual lenders. Two lenders can use the same label and offer different professions, loan limits, down payments, reserve rules, rates, and student-debt treatment.
Ask for the complete structure. The important columns are cash to close, monthly payment, reserves after closing, rate type, lender fees, mortgage insurance, and the time you expect to keep the home.
A future employment contract can be powerful and precise
Some programs allow eligible doctors to qualify using a signed employment contract before the job starts. The acceptable start-date window, contingencies, guaranteed income, credentialing, and reserve requirement are lender-specific.
Send the complete contract early. A base salary, bonus, call pay, productivity compensation, and forgivable recruitment loan are not always treated the same way.
Student debt needs lender-specific math
A large medical-school balance does not automatically prevent approval. The calculated monthly obligation can differ across physician, conventional, FHA, and VA guidelines.
Provide the current statement and repayment documentation. Deferred status, income-driven repayment, forgiveness plans, and a zero reported payment can each trigger different calculations.
Low down and no PMI still have a cost
A program that avoids separate monthly mortgage insurance can preserve cash and reduce one payment line. It may also carry a different rate or pricing structure than a conventional loan.
Compare the specialty program against a conventional, VA, or jumbo option using the same price and timeline. The winner is the loan that supports the larger financial plan, not the one with the best marketing phrase.
Match the loan to the likely career move
A physician early in training may expect a fellowship, attending role, partnership track, or another relocation within a few years. That changes how much to spend upfront to lower the rate and how heavily to prioritize cash reserves.
The home should still work if the expected timeline changes. Include moving costs, repairs, emergency reserves, and any student-loan payment change in the stress test.
Virginia market and property details still control the file
Condo approval, flood zones, property condition, acreage, mixed use, and appraisal all matter regardless of the borrower’s profession. The program also needs to be available in the property state and at the required loan amount.
